I was discussing this with a friend this evening. He is discipling some folks at his apartment complex and they have questions. In his case, he went to the altar and prayed the sinner's prayer. On his third trip, it "took". So how do you know if it has "took", so to speak.
For me, it was a few nervous years before I really knew that it "took". Why? Because although many people told me what to pray; that is, what words to say, no one explained the simple but really significant meaning of what I was praying.
In John 3:16, in the phrase "believeth on him (KJV)", believe means to "rely on" or "transfer your weight onto". In Romans 10:9, in the phrase "and confess with your mouth the Lord Jesus", the word "Lord" is key. If when you pray to receive Jesus as Savior and you also in your intent receive Jesus as Lord (Boss of your life), you become "born again". In receiving Jesus as Lord you transfer the responsibility of your life to Jesus. This also takes care of the "rely on" section of John 3:16. In receiving Jesus as Lord you are agreeing to live as He desires you to live.
How do you know it "took"? Check out your experience with 2Corinthians 5:17. The fundamental issue is about you being different (changed) after you ask Jesus to be you Savior and Lord.
